CT APRN SOCIETY NEWSLETTERJanuary 2023TOP ISSUESCTAPRN Society has been very active in advocating for meaningful health policy updates for the residents of Connecticut. As the APRNs of Connecticut have continued to provide high quality care throughout the pandemic, the importance of removing barriers to access of care have been even more visible to residents and legislators. In order to continue to gain traction, we need the voice of APRNsthroughout the state.Collaboration for New Graduate APRNsCurrently the state of Connecticut requires a new graduate APRN to have a collaborative agreement with a physician for 3 years and 2000 hours. This requirement is not evidence based and none of our surrounding states require this (NY, MA, RI, ME, NH).( Collaboration Issue Briefing)Solution: Update language to allow for new graduate APRNs to collaborate with experienced APRNs. Reciprocity for APRNs from Other StatesCurrently an experienced APRN from a different state would need a new collaborative agreement with a physician in order to practice in Connecticut. This is a barrier for many APRNs that want to work in Connecticut or open practices. (Reciprocity Issue Briefing)Solution: Update language to allow for experienced APRNs from other states to practice without a collaborative agreement if they have met the practice requirements.Global SignatureCurrently numerous Connecticut statutes and regulations were written before APRNs were in practice and donot list APRNs, despite their current scope of practice and training. This omits APRNs from signing forms for the care that they provide. (Global Signature Issue Briefing)Solution: Update language to allow for global signature authority for all forms or procedures within the current training and scope of practice of APRNs. THE LEGISLATIVE SESSIONThe Connecticut General Assembly Legislative Session for 2023 is underway. This is a long session with the opportunity to make meaningful change to health policy in thestate of Connecticut. The session will run from January 4, 2023 to June 7, 2023.

Regulation UpdatesNumerous regulations use language specific to physician regarding staffing and responsibilities, regardless of the qualifications of APRNs. These regulations cross various departments including DSS, DPH, and DMHAS. The regulatory process in Connecticut can be difficult to update. Solution: Continue to advocate for inter-professional updates to outdated regulations in order to reflect the qualifications of APRNs and to practice at the full extent oftheir training. “Thebody ofevidencesupports that therequirement for collaborativeagreementswithaphysiciana)decreasesaccesstocare b)decreasesthe quality of care,and c) increaseshealthcarecosts.”WANT TO GET INVOLVED?This will be a very busy legislative session and involvement from members across the state will be crucial. The best way for you to advocate is to talk to your legislators, patients, and colleagues about how these issues affect your own practice. You can print issue briefs and from the links to each topic and distribute them. MYTH VS FACT: DID YOU KNOW?Myth: APRNs need physician oversight to ensure high-quality care.Fact:DecadesofliteraturehavesupportedthatAPRNsarehighlytrainedandqualifiedtoindependentlydiagnoseandtreathealthcareconditions without a physician.Thebody ofevidencesupports that therequirementfor collaborativeagreementswithaphysiciana)decreasesaccesstocare b)decreasesthe quality of care,and c) increaseshealthcarecosts.After reviewing the quality of care and training, the National Academy of Medicine formally recommends against these barriers to practice.Myth:The requirement for a collaborative agreement with a physician is easy to get so isn’t a major issue.Fact:Numerous APRNs have been unable to find physician collaborative agreements for the practices in which they intended to work or were charged large fees (often > $10,000) for collaboration.TheBureauofLabor Statistics expectsthattherateofgrowthofAPRNwillbe45%from2020to2030,comparedto3%forphysicians.Therequirementforphysiciancollaborationisnotevidenced-basedandisnotsustainable.Myth:The forms that require a physician signature require it because the task is only best performed by a physician.Fact:When forms require a physician signature, usually the physician is not involved in the patient’s care in any way and has not seen the patient. APRNs independently diagnose and treat the patient without any physician involvement.

Increased staffing with nurse practitioners and physician assistants in geriatric primary care settings has been seen to result in improved access to care as well as improved quality of care. Thus, team-based practices serve an important role in quality geriatric primary care.

The FDA is accepting self-nominations until February 8, 2021 for multiple openings on advisory committees. Information regarding which committees are accepting nominations and the nomination process can be found in the FDA Notice (See link below). We encourage you, and any nurse practitioners that meet the requested criteria of any of the committees to self-nominate for those positions. As you know, it is important to increase NP participation on federal advisory committees. Please note that the self-nominations should go directly to the FDA in the process outlined in the notice.
